Upon submission of this form, no payment will be taken unless we have confirmed your location is suitable for dT Sky Wire and you have confirmed your payment details. Please remember that the dT Sky Wire technology does not require you to have a physical landline, so if you currently already have services contracted on your Landline, please remember to contact your provider to cancel the services if this is your intention (and check your permanence details or early cancelation fees if applicable). If you rely on your Internet connection, we recommend you do not cancel any services until dT SkyWire is up and running and to your satisfaction. Please remember that the obtainable speed depends on the distance from your property to the nearest tower. NOTE: Hibernation Facility can be enabled/disabled for full months from the 1st of the month to the end of the selected month. This service can be used after the first 2 full months of billing and not sooner. In order to enable Hibernation, this must be requested in writing at least 2 weeks before the end of the month. If you choose to have the Virtual Phone Line + Calls, we shall include a brand new local telephone number for you so that you can make and receive calls. Furthermore, I understand that Direct Telecom cannot be held responsible if certain IPTV services from 3rd party providers do not perform well. This process has a one time non-refundable setup fee of 99€ (105.93€ IGIC/ 119,80€ IVA) to process the application. We shall contact you to arrange for the payment once we have confirmed your location qualifies for dT SkyWire. Should the Installation Technician consider that there is no way to sucessfully install dT SkyWire at your location, you will be refunded the Setup & Installation Fee in full.

In order to achieve a successful dT SkyWire installation, please confirm the following. In the event of installations in communities, you must understand that you take responsability for the permissions to access roof/communal areas as well as cancellation fees & equipment removal fees should the community ask you to take it down.

I will have keys to access the roof on the day of installation if required.
Please agree to the following Installation conditions - (required for application)
* Engineers will usually install the equipment on an existing mast in your property. If this is absent, unsuitable or requires a special metallic bracket to guarantee a successful installation then this will be charged at 20€ + tax. * Up to 15 meters of cabling is included in the installation fee. If additional cable is required, this will be charged at 20€ + tax for every additional block of up to 10 meters. * Engineers are allocated up to 2.5 hours per installation which is usually more than enough. If additional time is required due to complexities caused by circumstances of the property etc, this will be charged at a rate of 35€/hour + tax. * If you request special/additional technical assistance to perform additional technical tasks (must be confirmed with engineer) or jobs that are not related to the provisioning of the Internet service (such as configuring other devices, configuring telephone sockets, extending cables internally etc), then this will be charged at 55€ + tax.

Preferred Installation Day - Monday to Friday.
We will try our best to try and fit in the installation on days that are most suitable for you. Please note that the the installation can be performed no sooner than a minimum of 48 hours after submitting this service application form and payment has been taken. We will therefore try to make the appointment for the soonest working day. Applications & Installations are only processed during working week days.

Preferred Installation Time Monday to Friday.
Please specify what time of the day would be most suitable for you in order to perform the Installation. We will try our best to make sure the installation takes place within your chosen timeframe if we can. This will be confirmed with you over the phone and via SMS. Please remember that once the appointment has been confirmed and accepted you must notify us if you feel you will not be able to attend the agreed installation timeframe within 24hours so that it can be rescheduled. Failure to do so will incur a "missed appointment fee" of 50€ and the appointment would have to be rescheduled for another day.
